Welcome to Myatts Field Medical Practice
At Myatts Field Medical Practice in London the doctors and nurses take pride in offering
the highest standard of patient-centred healthcare. We run many clinics for chronic disease care
and offer a wide variety of other medical services such as antenatal and postnatal care, minor
surgery, childhood vaccinations and well-person check-ups.
In addition to everything you need to know about the practice you will also find a wealth of

We have now settled into our new premises - Akerman Health Centre, 60 Patmos

WINTER VOMITING BUG
The symptoms of norovirus are very distinctive
– people often report a sudden onset of nausea followed by projectile vomiting and watery
diarrhoea.
Please help to curb the spread of norovirus

the link. Do not visit the GP as this is a self limiting virus which should clear up in 1 to 2 days.
OPENING TIMES OVER THE FESTIVE PERIOD
Surgery is open as normal on Monday 23rd of December and Tuesday 24th of December and on
New Years Eve Tuesday 31st December. However on Christmas Eve and New Years Eve
sample collections will be in the morning only and some other services may not be available. We
will be offering a walk in service along with telephone consultations and on the day booked
appointments on those days. Our usual Late night Monday appointments will be available but
those places are limited.

The reception is manned at the above times however the telephones are manned from 8:00 18:30 Monday to Friday. We do not close for lunch.
Out of Hours
We provide 24 hour emergency cover. If you are suddenly taken ill in the evening after 6.30pm
or on a weekend or Bank Holiday phone SELDOC on 020 8693 9066. SELDOC is the local
doctors co-operative, who will get a doctor to phone you back (please leave the phone free until
the doctor gets back to you).
He will then either give you advice, arrange for you to see a doctor at the SELDOC base:
Dulwich Hospital, East Dulwich Grove, SE22 or arrange a doctor to visit you, whichever he feels
is appropriate.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.

BOOKED APPOINTMENTS
We operate an appointment system for all afternoon/evening surgeries. Appointments may be
made personally or by phoning 020 3049 6733. If you find you cannot keep your appointment
please phone and cancel it. We have many ‘no shows’ in a week and this makes the wait for an
appointment longer. There are appointments available for emergencies every morning from 9
a.m to 11 a.m. Please call to confirm the emergency slots as they vary during times of annual
leave etc. In order to access your need it would be useful but not essential to let the reception
know what your reason for attending as an emergency is. Emergency slots are not for non
essential problems eg form filling.
APPOINTMENT REMINDERS
The practice will send SMS text message reminders of appointments please ensure we have your
correct mobile number for this service and keep us updated of any changes to your number. If
you wish you can opt out of this by letting us know.
EMERGENCY CONSULTATIONS
If you are very unwell and need to be seen before an appointment is available, tell the
receptionists and they will fit you in provided you present to reception before 11.00 am.
Emergencies will always be seen in the mornings but you may have to sit and wait and see the
first available doctor. These appointments are not for long term / non-urgent problems,
certificates or form signing.
EMERGENCY CONTRACEPTION
Available to all women, including teenagers, on request - please ask for an emergency
appointment
Home Visits
If possible please try to telephone reception before 11am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by
telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a
hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make
one home visit. There are also better facilities for examining and treating patients at the Health
Centre.
It is important that you keep us updated when you change address or phone numbers.
Test Results
Please phone in the morning for a call back between 2.00 and 3.00 for the results of tests. Blood
tests usually take about 5 working days to come back. X-ray and scan results usually take a
minimum of 10 working days to be sent to us.
Note that the practice has a strict policy regarding confidentiality and data protection and we will
only release test results to the person to whom they relate unless that person has given prior
permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the
practice.
It is your responsibility to check your results and to make an appointment to discuss them with
your doctor if your are advised to do so.
TELEPHONE ADVICE
If you wish to speak to a doctor for advice phone in the morning There are a limited number of
telephone appointments every afternoon.
The doctor will call you back as soon as they can. Please ensure we have the correct contact
details. It would be helpful to let the reception know the reason for a call back requests eg for
results etc. Please do not ask for a Doctor to call you in order to request regular repeat
medications as they should be ordered 2 working days in advance before you run out
of medications.
HSCIC
You may be aware that NHS England has agreed that the Health and Social Care Information
Centre can extract information on you in order to plan and improve services to support your
treatment and care. This information does not include your name but will include your NHS
number and postcode. If you wish to opt out of this please inform our reception team in order for
us to codes your records to ensure that information is not extracted. More information is
available on www.hscic.gov.uk (www.hscic.gov.uk) we also have leaflets in the reception
outlining why this information is required. Every patient is automatically opted in until we are
advised to remove them from the data extraction.
SCR
Summary Care Records is anelectronic record accessed only by other health care agencies eg
District Nurses, Hospitals etc of basic information i.e. current medications, allergies. You also
have the choice of opting out of this but need to let us know if you want to opt out. Further

leaflets are available from our reception.
REPEAT PRESCRIPTIONS
To request a repeat prescription please fax, send or hand in the right hand side of your old
prescription or a note indicating which medications you need. If you do not have your original
order slip then please specifiy the strength and dosage (eg 10mg twice a day) and what you are
taking the medicines for.
We require 48 hours notice to have your prescription ready.
We are unable to accept phone requests except from housebound patients. You can also order
repeat medications online.
Electronic Prescribing (eps2)
The practice offers electronic prescribing where prescriptions are sent direct to the pharmacist of your choice. This
negates the need to collect a printed prescription. Ask at your local pharmacist to register for this service.
Controlled Drug Prescriptions
If you are being prescribed controlled drugs, we have a system in place for the collection of repeat prescriptions. For
safety reasons, we will require signed authorisation from you if someone else is collecting the prescription on your
behalf and evidence of identity from the person collecting the prescription.
Other Information
Patients on Warfarin will need to provide their yellow book with an up to date INR reading. Patients on Hormones
(contraceptive or HRT) or Hypertensive medications wil need regular BP checks and may be advised to attend the
practice nurse.

From 1 April 2011, the charges are:



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. Telephone advice and order line 0845 850

Please ask your local Pharmacy if they offer this service which would enable you to seek
treatment from them for some illnesses.
The scheme covers most minor ailments, for example; indigestion, heart burn, stomach upset,
diarrhoea, constipation, sore throat, headache, earache, temperature, nasal congestion, hay fever,
head lice, thrush, and unprotected sexual intercourse.
Flu Vaccines arriving early October please arrange a nurse appointment if you are over 65 or
suffering from a long term condition eg Diabetes or Asthma or if you are a main carer for an
elderly or disabled person. If you are unsure whether you are elligible please contact our
receptionist team for advice.
Many pharmacist offer a vaccination service for around £15 to £20 for those that do not fall into
the target population.
Clinics & Services
PRACTICE NURSING
The practice nurses are available for blood pressure checks, dressings, blood tests, cervical
smears, immunisation, stitch removal. They will also give advice on giving up smoking, diet,
exercise etc.
MATERNITY SERVICES
Both ante-natal and post-natal care is provided together with our midwives who are based at the
health centre.
CONTRACEPTION
Contraception advice and services, including emergency contraception is available, from your
doctor. Post-coital contraception is available within 72 hours. (the earlier the better).
We do not fit coils but these can be fitted at Community Family Planning Clinics locally (list
available at reception) All contraceptive advice is confidential for all our patients - including
teenagers
WELL WOMAN CHECKS & CERVICAL SMEARS
Lambeth recommend that women aged 25 to 64 should have cervical smears every 3 years.
Please make appointment with the nurse.
COUNSELLING & PSYCHOLOGY SERVICE
You are able to see a well qualified counselor or psychologist at the practice by referral from
your doctor.
ASTHMA & DIABETIC CLINICS
The practice nurse runs these clinics by appointment.
MINOR SURGERY
The practice offers some minor surgery.
HOSPITALS
When necessary, we use the services at King’s College Hospital, St Thomas’ Hospital and Guys
Hospital.
TRAVEL CLINIC
We offer immunisations to those who require them. Please make an appointment to see the
practice nurse at least 2 to 3 weeks before you travel. There is a charge for certain
immunisations. All travel clinic appointments are subject to an administration charge (See
private charges).
CHILD HEALTH SURVEILLANCE
The practice provides baby clinics and childhood immunisations in conjunction with our health
visitor and community child health clinics . These are held at the health centre on Wednesday
afternoon and Friday mornings. If you need to see the doctor because your baby is unwell please
see the doctor during normal surgery hours.
CHILD IMMUNISATIONS
These are done during normal surgery hours or during baby clinics by the nurse. It is
recommended that children have immunisations against diphtheria, tetanus, whooping cough,
polio, meningitis C and HIB at 2 months, 3 months and 4 months and a pre-school booster
against polio, tetanus and diphtheria at about 3 years 6 months. Immunisations against measles,
mumps and rubella (MMR) are given at 1 year and a booster at about 3 years 6 months. A
Tetanus booster is recommended every 10 years.
STOP SMOKING CLINIC
We offer advice and support and can prescribe various aids eg inhalators, patches , gum etc.
Please ask at reception if you wish to make a DOUBLE appointment.
Sick Notes
Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is
available from your employer or at our reception or on the HMRC website
(www.hmrc.gov.uk/forms/sc2.pdf) .
For any illness lasting longer than seven days you will need to see the doctor for him/her to issue
a sickness certificate (F.med3) and for any subsequent renewal of the certificate. If your
employer insists on a sickness certificate for 7 days or less a charge will be levied.
Registration
New Patient Registration
All new patients are offered a general health check in the first month on joining the practice.
Please try to bring details of past medical problems, medication you are taking and smear and
immunisation history if appropriate.
Please bring a urine sample with you.
All patients over 75 are eligible for an annual check with the nurse. The practice recognises the
right of patient choice. Every effort will be made to give the patient an appointment with the
doctor/nurse of choice, but the waiting times may be longer. If interpreters are required we have
to offer appointments that are agreeable to them.
When registering children up to the age of 16 we require proof of parental responsibility eg birth
certificates or adoption papers.
Please confirm that you are in our catchement area before you complete online registration
forms.

Pre-registration with the practice
Once you have confirmed that you are in our catchment area, you pre-register click on the link
below to open the form. When you have completed all of the details, click on the "Send" button
to mail your form to us. When you visit the surgery for the first time you will be asked to sign
the form to confirm that the details are correct. We will need to see proof of parental
responsibility when registering children under the age of 16. Incomplete forms may be rejected
please contact us within 2 weeks of submitting a registration.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it
can take a considerable time for us to receive your medical records. There is an online version of
this file too, which you may fill out and send to us. When you come to the surgery you will be
asked to sign this form to confirm that the details are correct.

Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect.

PALS
PALS (Patient Advice and Liaison Service)
(www.pals.nhs.uk/) The NHS employs over a million staff in thousands of locations. It is a large
and complex organisation providing a broad range of services. It is not surprising that sometimes
you or a loved one may feel bewildered or concerned when using the NHS. And this can be at
times when you are feeling at your most vulnerable and anxious.
So, what should you do if you want on the spot help when using the health service? The NHS
expects all members of staff to listen and respond to you to the best of their ability. But
sometimes, you may wish to talk to someone employed especially to help you. The Patient
Advice and Liaison Service, known as PALS, has been introduced to ensure that the NHS listens
to patients, their relatives, carers and friends, and answers their questions and resolves their
concerns as quickly as possible.
PALS also helps the NHS to improve services by listening to what matters to patients and their
loved ones and making changes, when appropriate.
What does PALS do?
In particular, PALS will:

Provide you with information about the NHS and help you with any other health-related
enquiry
Help resolve concerns or problems when you are using the NHS
Provide information about the NHS complaints procedure and how to get independent
help if you decide you may want to make a complaint
Provide you with information and help introduce you to agencies and support groups
outside the NHS



Inform you about how you can get more involved in your own healthcare and the NHS
locally
Improve the NHS by listening to your concerns, suggestions and experiences and
ensuring that people who design and manage services are aware of the issues you raise
Provide an early warning system for NHS Trusts and monitoring bodies by identifying

Summary Care Records
From 2010 the NHS will be uploading records onto a database which will enable other NHS
bodies to access important information such as allergies, prescription. Once it is created it will
not be accessed without your permission. This upload will happen automatically to all patient

ACCESS TO MEDICAL RECORDS
The practice operates a strict policy of confidentiality, however certain information may be
shared with other NHS bodies as appropriate for the continued well being of the patient.
Patients may have access to their medical records by arrangement with their GP or the practice
manager. You may not remove your records from the health centre but may obtain copies for

Patient Responsibilties
Patient Responsibilities & Obligations
We aim to maintain a high standard of care but with just under 5000 patients registered at the
practice it is necessary for all patients to work with us.
You should always notify the surgery should you need to cancel or rearrange your appointment
as soon as you are aware you cannot attend to allow re-booking for our other patients.
If a doctor or nurse has asked you to return for another appointment or to be reviewed it is
important that you make a new appointment and remember to attend.
Remember an appointment is for one person only if other members of the family need to be seen
please ensure you have made them an appointment to give the Doctor or nurse time to treat them
effectively.
Wherever possible patients are expected to attend the surgery and turn up for their appointment.
Genuine housebound and seriously ill patients have a right to a home visit when necessary.
Please let the surgery know as soon as you change address or telephone number.
We expect patients to extend to staff the same courtesy that is given to them.
How We Use Your Records
We may have to share information about the care you receive with other health agencies in order
to plan and improve local services. You need to inform us if you wish to opt out of this. The
information will include your postcode and NHS number but not your name. For further details

Children's Immunisation Schedule
Here's a checklist of the vaccines that are routinely offered to everyone in the UK for free on the
NHS, and the age at which you should ideally have them.
2 months:



Diphtheria, tetanus, pertussis (whooping cough), polio and Haemophilus influenzae type
b (Hib, a bacterial infection that can cause severe pneumonia or meningitis in young
children) given as a 5-in-1 single jab known as DTaP/IPV/Hib
Pneumococcal infection

Seasonal Flu Vaccination
Influenza – flu – is a highly infectious and potentially serious illness caused by influenza viruses.
Each year the make-up of the seasonal flu vaccine is designed to protect against the influenza
viruses that the World Health Organization decide are most likely to be circulating in the coming
winter.
Regular immunisation (vaccination) is given free of charge to the following at-risk people, to
protect them from seasonal flu:








people aged 65 or over,
people with a serious medical condition
people living in a residential or nursing home
the main carers for an elderly or disabled person whose welfare may be at risk if the carer
becomes ill
Pregnant women
Children aged 2 or 3 (Fluenz Nasal Spray)
healthcare or social care professionals directly involved in patient care, and
those who work in close contact with poultry, such as chickens.
If you do not fall into any of the above categories and still would like a flu vaccine major
pharmacy stores offer the service at a cost of around £10 to £15.

Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is
available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of
medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate,
now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence
that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.
Your employer will decide whether or not this evidence is acceptable. If your employer has any
doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - ’Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help
you return to work sooner by providing more information about the effects of your illness or
injury.

Staff Details
We are a long established practice with its roots dating back to the 1970's when Dr Berlyn joined
a local GP (Dr Newman). The practice moved to its current location in the 1980's.
The practice is a typical family practice where we like to get to know the patients rather than just
treat their symptoms/illnesses. We offer same day appointments, home visits when needed and
offer extended hours.
Our staff turnover is very low (almost non existent) as we value and appreciate our staff. We
look forward to moving to our new premises in July 2012 though we will be sad to leave our
original "home". There is information on the new premises in newsletters which are posted
regularly on this site. The new premises will be known as Akerman Road Health Centre.

armed forces. He is known to all as Bob and his outside intrests
